Here's exactly what to expect will happen during a free home care assessment:
What Is a Home Care Assessment?
A home care assessment is a free, no-obligation conversation between you, your loved one and a member of our care team. Its purpose is simple: to understand your loved one's needs, preferences and daily routine so that we can recommend the right level of support for them.
It isn't a medical examination. It isn't a sales pitch. And it certainly isn't a commitment to anything.
It's simply the first step in building a picture of who your loved one is and what kind of care would genuinely help them, so that if you do decide to go ahead, the care plan we create is tailored entirely around them as an individual.
Who Conducts the Assessment?
At Homelium Care, your free care assessment is carried out by an experienced member of our care team, someone who understands both the practical and emotional sides of starting home care.
They'll be friendly, patient and completely unhurried. There's no script, no pressure and no expectation that you'll make any decisions on the day.
Where Does It Take Place?
In most cases, the assessment takes place in your loved one's home. This allows our team to get a sense of the environment, understand the daily routine and identify anything that might need to be factored into the care plan, whether that's mobility considerations, medication routines or simply where your loved one likes to sit in the morning.
If a home visit isn't possible or preferable, we're also happy to conduct the assessment over the phone or via video call, whatever feels most comfortable for your family.
What Questions Will Be Asked?
The assessment covers a range of areas to make sure we build as complete a picture as possible. You can expect questions around:
Daily routine and independence - What does a typical day look like? What can your loved one manage independently, and where do they need a bit of extra support? Are there specific times of day that are more challenging than others?
Personal care needs - Do they need help with washing, dressing, toileting or other aspects of personal care? How do they feel about receiving this kind of support, and are there any preferences around how it's delivered?
Medication - Are there medications to be managed? If so, what does the current routine look like and where does support need to come in?
Health conditions and medical history - Are there any conditions, such as dementia, Parkinson's, diabetes or mobility issues, that need to be factored into the care plan? Are there healthcare professionals already involved who we should be aware of?
Social and emotional wellbeing - Does your loved one feel lonely or isolated? Are there hobbies, interests or activities they'd like to continue? What matters most to them about their daily life?
Preferences and personality - What kind of person is your loved one? Do they prefer a structured routine or a more flexible approach? Are there particular things that matter to them, a certain time for their morning tea, a favourite TV programme, a weekly phone call with a grandchild, that a carer should know about?
Family involvement - How involved does the family want to be in the care plan? Are there specific concerns you have as a family member that you'd like to flag?
What Happens After the Assessment?
Once the assessment is complete, our care coordinator will put together a personalised care plan based on everything discussed. This sets out exactly what support will be provided, when visits will take place, what each visit will involve and who the carer will be.
You'll have the chance to review this plan in full before anything begins and to make changes if anything doesn't feel quite right.
If you decide to go ahead, we'll work with you to make sure your loved ones care package is right for them and make tweaks as the care continues to make sure everyone is happy. If you decide you're not ready yet, or that home care isn't the right choice right now, that's absolutely fine. There's no obligation and no follow-up pressure. You can always contact us again in the future once you feel ready.
How Long Does It Take?
Most home care assessments take between 45 minutes and an hour. We always allow plenty of time so the conversation never feels rushed, this is an important decision for your family and it deserves the time it needs.
What Should You Prepare?
You don't need to prepare anything formal before the assessment. But it can be helpful to have a think about:
What a typical day looks like for your loved one
Any medications they're currently taking
Any healthcare professionals or services already involved in their care
The key things that matter most to your loved one about their daily life
Any specific concerns you have as a family member
If you have a list of medications or a care plan from a previous provider, it's worth having those to hand, but they're not essential.
